SampleGateIDCard

Recent legislation has established the need for pesticide applicators and GATE card applicants to provide verification of lawful presence within the United States of America.

For renewal of the GATE card, producers will need their current GATE card number, an affidavit stating that they have lawful presence in the United States of America, documentation from the approved list to verify lawful presence, and the license fee.  The approved list is available at the website below and includes; valid U.S. State issued Driver’s License, Birth Certificate, U.S. Passport, or Permanent Resident Card.  A blank affidavit can be printed from the GATE webpage from the Georgia Department of Agriculture’s website. (https://forms.agr.georgia.gov/GATE/Default.aspx)  License fees are $20 for online renewal/application or $25 for assisted renewal/application.  Assisted renewal is considered application by mail or via the call number listed below.

When applying for the GATE card for the first time you will be required to provide the affidavit of legal presence in the United States of America, supporting documentation from the approved list (see above), and the license fee. Both renewals and new applications can be completed online at the address above.
